At the beginning of each semester, we need to collect certain information from you.  You can come into the office and provide it OR you can fill out this form and submit it electronically.  By participating, you will get into the habit of checking important information on a regular basis.  Also, we need to collect this information for your records and the USCIS (formerly the INS).

  5. Enter your SEVIS I-20 ID#   in the space provided below.

  6. Enter the date of expiration date of your I-20: (the last date in ITEM #5)
    If you are getting close to the expiration date of your I-20 and feel you will need an additional semester to complete your studies, the OISS can add an extension, but you will need to advise us of your situation.

  8. IS THIS YOUR LAST SEMESTER?? Meaning are you graduating at the end of it?
     Type the word YES in the box if it is, otherwise leave it blank
